The Complete Ministry Application takes approximately 30 minutes to complete. Please note that your progress does not save. If you are unable to finish the application in one sitting, you may click on the individual links and complete each section separately. Once you have submitted this form, please email a copy of your driver’s license to rachel.greenwall@ccofqc.org. After your license is received, you will be sent an email with instructions to complete the background investigation.

This Quick Start Application is not a full Ministry Agreement. Its purpose is to allow us to run a background check so we can legally and safely schedule you to serve in Children’s Ministry once per quarter. If you ever feel led to serve in a greater capacity, we would be happy to guide you through the Official Ministry Application process.
